Are there any side effects to black cohosh?

Hi,
I've just started taking black cohosh for PMT as I get bloated and very, very stressy before my period. I have found that the 3 types of pill which I have tried disagreed with me and I cannot take oestrogen as I have focal migraines.

I also take a mix of other supplements including vitamins a-d, milk thistle, spirulina, malt and Echinacea. I started taking milk thistle after reading in a book that if your tongue quivers or twitches when you stick it out, then you need to cleanse your liver and milk thistle was recommended. My tongue has a little line and a little whiteness and you have to really try hard to catch it quivering/twitching. I'm sure it looks better than before I started on the milk thistle. Maybe I am a bit paranoid!
I read (on the internet) that a woman had to have a liver transplant after taking black cohosh for only 3 weeks!!!

Are there side effects which you are aware of and should I avoid certain combinations?

Hi,

Black Cohosh is a herb that is primarily taken to relieve symptoms of the menopause, particularly hot flushes. It can also be used to alleviate menstrual cramps and PMT, which is the reason that you are taking it.

If you are taking any medical drugs, you should always consult a doctor before taking supplements. This is especially true of herbal supplements, as there are some known drug interactions. For example, Black Cohosh may intensify the effect of certain blood pressure lowering drugs and may interact adversely with female hormone products, such as the contraceptive pill. So, it is always worth checking with your doctor if you are on any prescribed medication.

In terms of side effects, Black Cohosh has been known to cause stomach upsets in certain people. A very high dose can cause nausea, vomiting and headaches, but this should not occur if you are following the recommended dose on the pack. One study also suggested that it may cause slight weight gain in some women, so be careful and monitor any side-effects you may have. Of course, you may not have any.

You are right that a few cases have been reported where severe liver failure has been attributed to Black Cohosh. This appears to be extremely rare, but there are signs of possible liver problems that you can be aware of if you are concerned i.e. nausea, loss of appetite, fatigue and tan-coloured urine.

You do seem to be more concerned than you need to be about taking supplements, if you don't mind me saying so! The trouble with taking lots of different supplements on an ad hoc basis, and regularly changing them, is that it is difficult to know what is benefiting your health. Maybe it's time for a more relaxed approach that focuses on a healthy diet and regular exercise routine?...
